Each participant enrolled in your group program must be provided with their own Breathe Well, Live Well® Participant Workbook and are provide 10 workbooks for free once fully trained. If additional workbooks are needed please place an order at Lung.org/PrintStore.

The American Lung Association has developed marketing and participant recruitment materials for Facilitators to use when implementing the Breathe Well, Live Well® program in their community. We ask all certified facilitators to notify the American Lung Association anytime you schedule a Breathe Well, Live Well® program, whether that be virtually delivered or in person. We also offer promotion of all community group programs at no cost on our Local Events website. If interested in promoting your group program, please complete the Breathe Well, Live Well Program Promotion Form. 

The American Lung Association is committed to ongoing evaluation of the Breathe Well, Live Well® program and requests that facilitators complete surveys to provide feedback and insight into the implementation of the program. It is also important for facilitators to administer the participant evaluation surveys to participants after the last session in order to understand the efficacy of the program.

As a Certified Breathe Well, Live Well® Facilitator you must report information about your program using the Breathe Well, Live Well Program Report Form (preferably within two weeks of completing the final program session, Section 5). Information collected in this form allows us to evaluate the effectiveness of our program’s delivery and reach.

Certified Facilitators are required to recertify every 3 years. The American Lung Association will notify you when it is time to recertify. The cost to recertify is $100. This fee may be waived if certified facilitators have conducted at least one program during their 3-year certification period and submitted the Facilitator Program Report Form.
